2. Expiration
Gift cards are valid for 12 months from the date of purchase unless otherwise specified at the time of issuance. Expired gift cards will not be honored or reissued.
3. Non-Refundable Policy
All gift card purchases are final sale. Gift cards are non-refundable and may not be exchanged for cash, credit, or other forms of payment.
4. Appointment & Redemption Policy
Gift cards must be presented at the time of service. All appointments booked using a gift card are subject to our standard cancellation policy: 24-hour notice required for cancellations or rescheduling Late cancellations or no-shows will result in a forfeiture of the gift card value applied to that appointment .
5. Partial Use
If the full value of the gift card is not used in a single visit, the remaining balance will stay on file for future use within the valid period.
6. Lost or Stolen Gift Cards Stay Young Health & Wellness Institute is not responsible for lost, stolen, or unauthorized use of gift cards. Replacement may be issued at management’s discretion with proof of purchase.
7. Promotions & Packages
Gift cards may be used toward promotional services or packages unless otherwise stated. Certain exclusive offers, memberships, or bundled pricing may be excluded at the discretion of management.
8. Medical Services Disclaimer
Gift cards may be applied toward eligible treatments; however, all services are subject to medical evaluation and provider approval. No treatment is guaranteed.
9. Right to Refuse Service
Stay Young Health & Wellness Institute reserves the right to refuse service if a treatment is deemed medically inappropriate or unsafe.
10. Policy Updates
Stay Young Health & Wellness Institute reserves the right to modify this policy at any time without prior notice.
** Gift cards issued by the practice for marketing, referral incentives, or promotional campaigns must be approved in advance by Dr. Farooqi.
All promotional gift card distributions must be documented, including recipient, date, occasion, and card value.
Gift cards may not be issued as incentives for patient referrals in a manner that violates the federal Anti-Kickback Statute. Promotional use should be reviewed against applicable regulations before distribution. Promotional gift cards may not be redeemed for cash and are valid only for services or products offered by Stay Young Health & Wellness Institute unless otherwise specified.
